PCN Pharmacist – The Millwood Partnership

As lead practice for the Gorleston Primary Care Network The Millwood Partnership is looking for a PCN Clinical Pharmacist to support Gorleston PCN in delivering the PCN contract. For this year this will mean working with care home patients to provide medicines support and Structured Medication Reviews as well as keeping abreast of other initiatives and PCN wide opportunities. The role will develop as the contract changes from year to year.
It is expected that you will work as an interface for the network practices with other agencies particularly community services, community pharmacy and secondary care. You will also report to the network Clinical Director and management lead and keep the network practices appraised of progress as well as working closely with the assigned practice leads for each care home.
The Millwood Partnership is a forward thinking practice in Bradwell close to the east coast of Norfolk. It is lead practice for Gorleston Primary Care Network, with the Clinical Director and Management Lead both part of the practice team.
Clinically well-staffed we have 9 partners and 2 salaried GPs to cater for a diverse population of around 19,000 patients. The practice is led by the Partners and team of three managers, with a support staff including nurse practitioners, practice nurses, HCAs, pharmacists, paramedic, administration, prescription clerk and receptionists. We are lead practice for our Primary Care Network and are a teaching and training practice to medical students and GP Trainees respectively.
