The list of prescribed medical certificates that a practices is contractually obligated to provide is available in Schedule 2 of the GMS regulations and also detailed on the BMA’s webpage ‘Certificates GPs cannot charge fees for’.
Forms not listed on these webpages do not form part of your GMS contract so the practice is not obligated to complete them and can decline the request if they choose to. If the practice chooses to complete other forms a reasonable fee can be charged for doing so. It is also worth highlighting the reports requested by Local Authorities that can be claimed for under collaborative arrangements from the ICB. More information about these forms/reports can be found here.
